Galenic syndromes: combinations of mental state and personality disorders too closely entwined to be separated

Many mental disorders are linked to personality, but this is rarely recognised in clinical practice. It is suggested here that when the links are very close, the two can be joined. Galenic syndromes are so named because Galen was the first physician to recognise the links between personality and disease.
Specialist teams as constituted are unsatisfactory for treating people with personality disorders

It is now becoming standard practice in most advanced economies to provide specialist services for those with personality disorder. Such services, almost exclusively, provide complex well-structured psychological interventions lasting many months for a small number of those with borderline personality disorder pathology. The evidence suggests that these treatments are effective but they can only be provided for a small number of people. However, in every area the numbers of patients with significant personality disorder far exceeds those that are treated, and most of these have other personality disorders. It is argued that the current service system is not working efficiently and should be replaced by one that provides resources and expertise within community teams with some external advice from specialists but no transfer of responsibility to a designated team.

Background
There is limited empirical information on service-level outcome domains and indicators for the large number of people with intellectual disabilities being treated in forensic psychiatric hospitals.
AimsThis study identified and developed the domains that should be used to measure treatment outcomes for this population.
MethodA systematic review of the literature highlighted 60 studies which met eligibility criteria; they were synthesised using content analysis. The findings were refined within a consultation and consensus exercises with carers, patients and experts.
ResultsThe final framework encompassed three a priori superordinate domains: (a) effectiveness, (b) patient safety and (c) patient and carer experience. Within each of these, further sub-domains emerged from our systematic review and consultation exercises. These included severity of clinical symptoms, offending behaviours, reactive and restrictive interventions, quality of life and patient satisfaction.
ConclusionsTo index recovery, services need to measure treatment outcomes using this framework.

Background
Previous studies of borderline personality disorder report neuropsychological impairments in several domains, including memory. No studies have compared memory functioning in high-risk prisoners with borderline personality disorder with similar prisoners with other personality disorders.
AimsTo explore mnemonic impairments in prisoners undergoing personality assessment as part of the dangerous and severe personality disorder initiative or detained in a medium secure facility.
MethodWe investigated memory function in 18 prisoners with borderline personality disorder and 18 prisoners with other personality disorders.
ResultsPrisoners with borderline personality disorder exhibited a pattern of multi-modal impairments in the immediate and delayed recall of verbal and visual information, with some association with affective instability. These deficits were not associated with the severity of personality disturbance.
ConclusionsThese data suggest that memory deficits have some specificity in relation to the constituent traits of borderline personality disorder and indicate that neuropsychological assessment may be a source of useful adjunctive information for distinguishing between the cognitive and psychological difficulties of individual prisoners.

The mechanical properties of thin-film Cu/Cu-Ni and Cu/Ag multilayers are examined in this study. Films were prepared by magnetron sputter deposition onto cleaved NaCl substrates. The use of a computer-controlled deposition system enabled the multilayer interface width to be varied, as well as the multilayer amplitude and bilayer thickness. The Cu/Cu-Ni multilayers had the form of (001) oriented single crystal films, but for Cu-Ag multilayers only polycrystalline films could be obtained in the (001) orientation. For Cu/Cu-15%Ni multilayers, the tensile strengths were measured and observed to increase with decreasing interface width at constant wavelength. Tensile strengths were also found to increase with increasing composition modulation amplitude. For films with constant interface width but bilayer thicknesses ranging from 10 to 60 nm, the tensile strength was relatively constant. The results are consistent with previous theoretical treatments, particularly the theory of modulus hardening.
